Home Care

The Home Care Services Program (HCSP) offers access to Medicaid-funded long-term care programs designed to help eligible elderly or disabled individuals remain safely at home, rather than in a nursing home or other institution. In some cases, the Home Care Services Program will assess medical needs and determine the appropriate care required. The HCSP offers access to the following long term care programs:

  • Personal Care (Medicaid-Funded Home Care) – home attendant and housekeeping services for people having difficulty with at least one or more activities of daily life, such as walking, cooking, cleaning, bathing and using the bathroom.

  • Long-Term Home Health Care Program – home care, case management, and skilled services for people requiring a higher level of care (skilled nursing home-level care at home).

  • Managed Long-Term Care Program – case management, skilled services and home care services for people who are Medicaid-eligible, or eligible for both Medicaid and Medicare, and who are medically eligible for the level of care normally provided in a health related facility or skilled nursing facility.

  • Assisted Living Program – long-term residential care, supervision, and home health care services.

  • Care-at-Home Program – medical and related services for severely disabled children who would otherwise be required to remain institutionalized.

Certified Home Health Agencies (CHHAs) are the responsibility of the New York State Department of Health (SDOH), not the City of New York. HCSP is responsible for transferring a client from a CHHA to Personal Care services when a CHHA so requests. Learn more about obtaining CHHA services.
