Help Is In Your Neighborhood

If you're having trouble making ends meet you may be eligible for benefits and services that stretch your dollars.

Now you can file an application for food stamps or Medicaid at a community organization in your neighborhood.

A staff of experts is available to assist you with submitting your application, as well as the necessary documents.

For your convenience, you can also ask for a telephone interview with the Human Resources Administration to complete the process at the time of your visit.

Then, you can stop by your local Food Stamp office to quickly confirm your identity, with very little waiting time.

Organizations in all five boroughs can help.

LIST OF DOCUMENTS YOU MUST BRING TO YOUR APPOINTMENT

  • Proof of Identity: 
    Photo ID, driver's license, passport, naturalization certificate, hospital/doctor's records, or adoption papers.
     

  • Proof of where you live: 
    Statement from landlord/ primary tenant, current rent receipt or lease, or mortgage records.
     

  • Proof of Income (if any): 
    Current pay stubs/envelopes, tax and/or business records, unemployment insurance or current income tax return.
     

  • Proof of Citizenship or Current Alien Status: 
    Birth certificate, U.S. passport, USCIS documentation, military service records, etc.

If applying for Medicaid, please also bring all medical bills up to 3 months prior to the application as well as the following:

  • Proof of other insurance or Medicare:
    Insurance benefit card or the policy or Medicare Benefit Card.
     

  • Additional proof of unearned income:
    Social Security, Supplemental Security Income (SSI), Veteran's benefits, retirement/ pension, or any bank books and insurance policies you may have. Bring a letter of support if there’s no income.
