Each of the forms below is available for download as a PDF document, and requires the free Acrobat Reader for viewing and completion.
Please fill out the downloaded permit forms by typing directly onto
the form from your computer (handwritten permits are not accepted, and
review permit samples for guidance). Print an original for signature
and submit in person with a photocopy of your valid photo ID to our office:

Your permit is valid ONLY with an original signature and an
official seal from a permit coordinator at the MOFTB.
For filming in city parks, please visit Parks Film Shoot Request Form and complete online form. Your request will be routed to the appropriate individual park manager. Please allow at least 48 hours for park manager to respond to your request. Once park manager has approved your shoot, submit MOFTB permit form below referencing park manager who provided approval.
Please Note: Filming interiors within city buildings,
bridges or tunnels will require additional permissions from the
controlling entities. Please call the MOFTB to obtain specific
contact information.
NEW! The MOFTB now requires all Motion Picture/Television Permit applications to provide a signed "Keys to the City" directive. This form may be downloaded below. Producers or Production Managers must read and sign. The original signature page must be submitted to MOFTB with the Motion Picture/Television Permit. Please keep a copy of the entire document for your records, and distribute copies to all crew members.
Please notify the community board, city council, block and merchant associations in which you are filming at least 48 hours in advance of your shoot.
