OTCR1 Alternative Material Acceptance Criteria Application
The OTCR1 is used to initiate review of existing standards and/or evaluation criteria for products categorized as alternative to the code. OTCR staff will manage the review process for adoption of DOB approved acceptance criteria.  Approved acceptance criteria are used by third party agencies for testing and/or certification (if required).

OTCR2 Site-Specific Approval Application
The OTCR2 is used to initiate the process of filing for a site specific approval. The Testing Laboratory of Professional Engineer performing the required test must also complete the OTCR3.

OTCR3 Testing Laboratory or Professional Engineer Statement
The OTCR3 is used by the Testing Laboratory or Professional Engineer to document required testing for the review of site specific approval.
