1) What is the difference between the filing year and the reporting year?

The filing year is the year in which you file your report. The reporting year is the calendar year to which the report pertains. For example, you file your 2003 financial disclosure report (reporting year) in 2004 (filing year).

2) I retired the end of 2003, but my accrued annual leave kept me on payroll into 2004. Will I be required to file for 2004?

No. You will be required to file only for the portion of 2003 in which you were employed for the City.

3) Is the information contained on the cover page of the report disclosed to the public?

No, this information is strictly confidential. Your social security number, home address and telephone number, and names of immediate family members are never released to the public.

4) I received overtime pay and longevity pay in addition to my regular salary. How would I report these payments?

You must report the total value by category and also list each type of payment in response to Question 1. For example, you received $60,000 in salary and $10,000 in overtime payments. Under "Nature of Income" state ""salary and overtime payments" and report the total amount as ""D" under ""Category of Amount of Income".

5) I received $750.00 for part time work from MACY's Department Store. Do I have to report this income?

No, any income under $1,000.00 need not be reported. However, you must report any non-City employment, regardless of the amount earned, in response to Question 2 (Identification of Your Non-City Employer or Business).

6) I am a licensed real estate broker but have not sold real estate for the past 2 years. Do I report this information?

Yes, in response to Question 3 (Regulated Professions). You should indicate that you are a licensed real estate broker but did not engage in such practice during the reporting year.

7) I am a member of the Knights of Columbus. Do I have to report this in response to Question 4 (Positions Held by You)?

No. You are not required to report membership positions.

8) Do I have to report unemployment benefits or an income tax refund as earned income?

No, unemployment benefits and income tax refunds do not have to be reported.

9) How do I report bank interest?

If you received bank interest of $1,000 or more, you must report it in response to Question 6 (Your Non-City Income). Under "Source" write the name of the bank, and under "Nature" write "bank interest". Indicate the "Category Of Amount" by writing the letter that applies to you.

10) I received $10,000 from a friend's life insurance. Must I report this?

Yes. You must report this income in response to Question 6 (Your Non-City Income).

11) Do I have to report reimbursements and/or payments for travel related expenses by a governmental entity?

Reimbursements and direct payments for travel related expenses by a governmental entity are not reportable, whether they are for City-related travel or not. Governmental entities include both domestic and foreign governments.

12) AT&T reimbursed me for my trip to California for City-related business. Am I obligated to report this?

Yes. Since AT&T is not a governmental entity, you would report this in response to Question 8. (Reimbursement of Your Expenses).

13) What if AT&T paid directly for my expenses or if the trip was not related to city business?

In both cases, this would be reported as a gift in response to Question 9.

14) Where do I report my NYC Deferred Compensation Plan?

You report this information in response to Question 14 (Your Interests in a Trust, Estate, or Beneficial Interest) NOT Question 7 (Your Deferred Income). Under "Identity of Interest" write "NYC Deferred Compensation" and under "Nature of Interest" write "Owner".

15) I am the beneficiary of a savings account established by my mother. Am I obligated to report this?

Yes. You are obligated to report this in response to Question 14 (Your interests in a Trust, Estate, or Beneficiary Interest). Under "Identity of Interest", write bank account and under "Nature of Interest", report that you are the beneficiary.

17) I own several different stocks, which are held in a custodial account with Merrill Lynch. Where and how do I report these stocks?

You report stocks in response to Question 16 (Your Securities). When reporting stocks you must indicate the company which actually issues the stock, not the name of the custodian. For example, you own $5,000 in IBM stock held in your Merrill Lynch custodial account. Report "IBM" as the "Issuing Entity" and under "Type of Security" write "Stocks". Indicate A as the "Category of Value of Your Interest". If you do not know the "Percentage of Corporate Stock Owned or Controlled" indicate unknown.

18) I have a regular savings account at my local bank. Am I obligated to report this in response to Question 16?

No. Regular savings accounts are not reportable.

19) I own a mutual fund through Fidelity. Where and how do I report this?

Mutual funds are reported in response to Question 16 (Your Securities). Report "Fidelity" as the "Issuing Entity" and "Mutual fund" as the "Type of Security". If you own more than one mutual fund with Fidelity, you should group them together for purposes of determining their value.

You are NOT required to report mortgages obtained to purchase your primary and secondary residences. Additionally, you do not have to report a second mortgage obtained to finance improvements to primary and secondary residences.

24) I have two credit card accounts with one bank with balances totaling over $5,000. Do I report both accounts?

Yes! You should report the name of the bank as the source and indicate the category of amount in response to Question 19 (Money You Owe).
